Registering with SAS
If you would like to register with Student Accessibility Services, please request an intake appointment online.

To register with SAS, please visit the SAS Student Portal, select “Request an Intake Appointment” and complete the registration form.

- You will receive a telephone call or email to schedule your intake appointment within 2 business days.

You will receive an email confirming the date and time of your intake appointment, including an Assessment of Functional Impairment (AFI) for you to complete.
- To prepare for your intake appointment, please upload your Assessment of Functional Impairment (AFI) and medical documentation (refer to note below) to “Upload Documentation” on the Student Portal.
- Important Note: Please fill out your medical documentation in Adobe Acrobat, as the signing function will only work in Acrobat.
- To open this file in Acrobat: right click the medical documentation link and select “Save Link As” then “Adobe Acrobat” (PC) or “Download Linked File As” (Mac). Save to your desktop and fill it out.

During this appointment, you and your Program Coordinator will review the accommodation process and complete assessment to determine academic accommodation.
No-Show Policy for Booked Appointments
- Please attend your appointment on time, to allow for a full assessment of your accommodation requests.
- If you are unable to attend your scheduled appointment, please provide 24-hour advance notice.
